The first thing that I noticed is that your right hand is catching (moving) to the outside of your body. This is a less advantageous angle for your hand/arm to be in and will decrease your efficiency. Focus on bringing your right hand back to the centerline. Secondly your right hand entering almost the same time as your elbow. I want you to try and enter fingertips first with a high elbow to set up the rest of your stroke in a proper position. Lastly, I want you to try and decrease the size of your kick. You should try and maintain a kick that is about the size of a shoebox, with small bubbles on the surface. Once you can get that, work on making it consistent throughout your stroke (maintaining the small kick when you breath).
